Medical & Religious Exemptions

Requesting an Exemption

You may request an exemption from the vaccination requirement based on verified medical reasons or religious beliefsRead the Board Policy on vaccinations.

For guidance on whether you meet the criteria for a Medical Exemption, please review the CDC and State of California guidelines

There is a separate form and process for each type of request. See details below for your exemption type.

 

Medical Exemption Request

  1. Download the COVID-19 Request Medical Exemption PDF Form 

  2. Complete the PDF form on your computer and have it signed by your medical provider.

  3. Log in to MyPortal.

  4. Navigate to Apps.

  5. Click on the Foothill College Student Health Portal tile under the Student section.

  6. Click on the "Document Upload" link in the navigation bar.

  7. Select Document Type "COVID-19 Medical Exemption Request." 

  8. Upload your completed PDF medical exemption form.

  9. You will receive an email with a decision from the Student Health Center within 2-3 business days.

  10. Your MyPortal account will be updated within 2-3 business days after your exemption has been approved.

Religious Exemption Request

  1. Download the Religious Exemption Request PDF form

  2. Complete the Religious Exemption Online Submission form and attach your completed PDF to the "File Upload" option.

  3. You will receive an email with a decision from the Dean of Students within 2-3 business days.

  4. Your MyPortal account will be updated within 2-3 business days after your exemption has been approved.

Healthcare providers or health departments in the United States can request a consultation from CISA COVIDvax for a complex COVID-19 vaccine safety question that is (1) about an individual patient residing in the United States or vaccine safety issue and (2) not readily addressed by CDC or Advisory Committee on Immunization Practices (ACIP) guidelines. If you feel you have a medical reason that requires a medical exemption that is not listed as a contraindication, please have your doctor consult with CISA COVIDvax

 

Testing Compliance Update for Exempted Students

Due to recent changes from Santa Clara County (SCC) and World Health Organization (WHO), Foothill College no longer requires COVID testing for students who have an approved medical or religious exemption on file with the college. This requirement has been removed effective Monday, September 26, 2022.
