COVID-19 Health & Safety

Vaccination Policy Update

On Monday, April 3, 2023, the Board of Trustees approved the rescission of Board Policy 3507 COVID-19 Vaccination Interim Policy. Effective July 3, 2023, the first day of Summer 2023 Quarter, COVID-19 vaccinations will no longer be required at Foothill-De Anza Community College District. The lifting of the COVID-19 vaccination mandate applies to all faculty, staff, students, vendors, and visitors. Individuals will no longer need to provide proof of COVID-19 vaccination to access in person district programs and services. Please note the COVID-19 vaccination mandate will remain in place through July 2, 2023

Spring Quarter: Uploading Proof of Vaccination

If you plan to take in-person classes

  • You must upload proof of vaccination before registering.
  • If you've already uploaded your proof previously, you do not need to resubmit.

Fully vaccinated means you received either two doses of the Pfizer or Moderna vaccine or one dose of the Johnson & Johnson (Janssen) vaccine.

We've made it easy to upload a copy of your vaccination card using the Foothill College Student Health Portal. See the guide below for steps.

Submitting an Exemption

To request an exemption, please refer to the Medical & Religious Exemptions webpage. 

Student Health Portal Guide

Accessing the Student Health Portal

  1. Log in to MyPortal.

  2. Navigate to Apps.

  3. Click on the Foothill College Student Health Portal tile under the Student section.

  4. If you receive an error message, allow up to 48 hours from your registration date for your Student Health Portal to become active. 

Submitting Proof of Vaccination

  1. Log in to MyPortal.

  2. Navigate to Apps.

  3. Click on the Foothill College Student Health Portal tile under the Student section.

  4. Select “My Forms” from the top menu.

  5. Choose "COVID-19 Vaccine Submission." If this form is not listed, use the "Document Upload" feature.

  6. Enter the appropriate information (Note: Lot number is not required for submission).

  7. If you have it, upload a photo or digital screenshot of your vaccine card.

  8. Click “Submit” once. At the top of the page, you will see a “Thank you for your submission” note. To verify your upload, refer to the "Verifying COVID Vaccine Approval" instructions below.

  9. Allow 4-6 business days for your registration hold to be released.

  10. Register for courses.

Verifying COVID Vaccine Approval

  1. Log in to MyPortal.

  2. Navigate to Apps.

  3. Click on the Foothill College Student Health Portal tile under the Student section.

  4. Select the "My Profile" link from the top menu bar.

  5. Select "Immun. History" from the drop-down menu.

  6. A PDF will download with your COVID-19 status, general vaccine history, and vaccine exemptions.

  7. If you do not see your COVID vaccine or exemption dates, your vaccine records have not been processed. This may take 3 business days.

Verifying Document Uploads

  1. Log in to MyPortal. 

  2. Navigate to Apps. 

  3. Click on the Foothill College Student Health Portal tile under the Student section. 

  4. Select the "My Profile" link from the top menu bar. 

  5. Select "Images" from the drop-down menu. 

  6. You should see a listing of images uploaded to the Student Health Portal. 

  7. If you do not see your document uploads, please reupload 1 time.  If you still do not see your upload, please email covidquestions@fhda.edu. 

Submitting a Medical Exemption Request

  1. Download the COVID-19 Request Medical Exemption PDF Form 

  2. Complete the PDF form on your computer and have it signed by your medical provider.

  3. Log in to MyPortal.

  4. Navigate to Apps.

  5. Click on the Foothill College Student Health Portal tile under the Student section.

  6. Click on the "Document Upload" link in the navigation bar.

  7. Select Document Type "COVID-19 Medical Exemption Request." 

  8. Upload your completed PDF medical exemption form.

  9. You will receive an email with a decision from the Student Health Center within 2-3 business days.

Submitting Informed Consent

  1. Log in to MyPortal.

  2. Navigate to Apps.

  3. Click on the Foothill College Student Health Portal tile under the Student section.

  4. Select “My Forms” from the top menu.

  5. Click on "Informed Consent - Foothill College Students Only"

  6. Complete the form.

  7. Click on the "Submit" button to finish.

Submitting Proof of MMR

(Stanford Students Only)

  1. Log in to MyPortal.

  2. Navigate to Apps.

  3. Click on the Foothill College Student Health Portal tile under the Student section.

  4. Click on the "Document Upload" link in the navigation bar.

  5. Select Document Type "MMR Vaccination - Stanford Students Only." 

  6. Upload proof of MMR vaccination.

  7. Click the "Save" button to submit.

Submitting Proof of Medical Insurance

(Stanford Students Only)

  1. Log in to MyPortal.

  2. Navigate to Apps.

  3. Click on the Foothill College Student Health Portal tile under the Student section.

  4. Click on the "Document Upload" link in the navigation bar.

  5. Select Document Type "Medical Health Insurance - Stanford Students Only." 

  6. Upload proof of medical health insurance.

  7. Click the "Save" button to submit.
