Get Your Free Flu Vaccine On Campus

Who: Open to all students, faculty, and staff

When: Tuesday, Oct. 25 at 10 a.m. –2 p.m.

No appointment required. The flu vaccine clinic is in partnership with Walgreens on a "first come first serve basis" and subject to availability, so please arrive earlier to ensure a ready supply.

Thank you to Angelu Su, our Pharmacy Technician program director, for organizing this free flu vaccination clinic on campus with Walgreens. Even though COVID 19 is still a concern, the flu can be very dangerous as well, so don't delay. Get yourself vaccinated.

What to Bring


  • Please bring your insurance card (or digital equivalent) if you have one; if not, you'll just need to fill out a vaccine voucher form.
  • You will be asked to fill out a short Vaccine Administration Record form (name, address, physician, insurer, etc.) so that Walgreens can send the vaccination information to your primary care provider.
  • You may complete and print the form to bring with you. If not, there will be forms available on site.

Do I need to make an appointment? 


No, this Flu vaccine clinic is "first come first serve."
 

How much does it cost?


Individuals without health insurance: FREE (must bring State issued Driver's License or ID)


Individuals with health insurance: Should be covered by your health insurance. Please bring your health insurance card and Walgreens will bill them directly.

How long will the entire process take?

Please plan for at least 15-20 minutes as you will need to complete a short Vaccination Administration Record (VAR) form and then receive your vaccine with a pharmacist.

It may take longer depending on the number of individuals waiting for their vaccination and the time of day. 

Where can I get the Vaccination Administration Record (VAR) form?


It will be available at the check-in station in front of Building 5200: You can fill out the VAR form onsite. 


Program Director/Instructor can obtain the copies of the form in the Exam Filing Cabinet in 5200 (bottom drawer of the filing cabinet)

Will completing the VAR form BEFORE arriving at the clinic help expedite the process?


Yes, as you can directly proceed to the "VAR Processing Station" to be cleared to receive your vaccine.

Do I have to complete the entire VAR form?


No, only complete the "highlighted" yellow sections of the form.

What if I don't know how to answer a question on the VAR form?


Leave it blank, we can help you at the "VAR Processing Station" when you arrive for your vaccine.
