Student Health Records
Students who received health services at the Main Campus Health Center after September 25, 2017 can access their Electronic Medical Records (EMRs) in two ways.
For vaccines and covid related items: PyraMED in MyPortal
For health visits: Use the Athena/Instant Urgent Care portal (IUC)
Prior to June 30, 2017
Planned Parenthood was our contracted provider for health services prior to June 30, 2017. Records may be retrieved through Planned Parenthood in Mountain View.
To access your records, you'll need to complete a records request form in person.
